When even two proofreaders are not enough

Even established newspapers can fall prey to the dreaded typo.

Consider this: A Charlotte Observer reporter wrote a short sports article on a sports player who was plagued by a herniated disc. The story was checked by an editor and then a second editor. The second editor, noting that “disc” didn’t conform to the paper’s style guidelines, changed the spelling to “disk.” Or so he thought.

Even with two proofreaders, mistakes still happen — sometimes resulting in hilarious consequences.
